The consent form can be found at ludingtonartscenter.org or LACA during business hours on Tuesday, Thursday, and Friday from 11 a.m. to 4 p.m., Wednesday from 11 a.m. to 6 p.m., and Saturday from 11 a.m. to 2 p.m. LACA is located in downtown Ludington at 107 S.
